Physical Therapist

**Job Summary**- Interviews patients and their relatives/companions regarding present and past history of illness.- Keeps accurate and complete records of patient's data, PT evaluation, progress notes and treatment programs.- Carries out physical therapy procedures as prescribed by the physiatrist or orthopedic doctors and accredited physiatrists outside of OLLH.- Clears with the physiatrist all direct referrals before carrying out prescriptions.- Gives home instructions for follow-up therapy program and self-care activities as prescribed by the physiatrist.- Records physical therapy evaluation and makes subsequent progress notes.- Accomplishes all clinical data which are directly referred to Physical Therapy.- Transports in-patient from room to PMRD and vice-versa.- Edits the initial evaluation, re-evaluation, progress and discharge notes and endorsements of the assigned intern to him/her.**Job Qualifications**- Graduate of Bachelor of Science in Physical Therapy- PRC Board Passer- At least 6 months related work experience**Benefits**:- Paid trainingSchedule:- 8 hour shiftSupplemental Pay:- 13th month salary- Overtime payAbility to commute/relocate:- Manila: Reliably commute or planning to relocate before starting work (required)**Education**:- Bachelor's (required)License/Certification:- Physical Therapist (required)
