KEY RESPONSIBILITIES AND DUTIES1. Conduct a situation assessment in the assigned area to gather information on the number of private health facilities and stand-alone physicians, existing professional organizations and TB services for the screening, testing & diagnosis, treatment, prevention both public and private and update mapping done at least once every six months for possible changes.2. Assist in the planning and conduct of the orientation and capacity building of mapped stand-alone physicians on TB screening, testing & diagnosis, treatment, prevention and reporting of TB cases diagnosed using ITIS.3. Provide a location map of private hospitals, stand-alone clinics and laboratory facilities mapped; including availability of transport for the selection of possible facility that will be utilized as referral facility for free x-ray and sputum collection; monitor service utilization and identify barriers to effective service provision and facilitate payment of fees for services provided.4. Prepare a monthly monitoring plan indicating the schedule of visits to engaged hospitals and stand-alone private physicians to monitor implementation progress and collect data on project indicators. Assist private physician register to ITIS and report TB cases diagnosed including the outcome of treatment; facilitate referral of patients for free x-ray and gene Xpert including access to free medicine; take note of issues in mandatory notification encountered and coordinate with the Cluster Coordinator and the NTP on possible solutions to challenges identified.5. Maintain the NPO Mandatory Notification Database which contains all the information related to the implementation matrix indicators gathered from either the following sources: MOP Form 4A-TB Notification Form, the Facility TB Registry or Hospital TB Referral Logbook or the Clinical Patient Card maintained by the stand-alone physician which in compliance with the Data Privacy Law remain at the facility or the clinic of the physician. Facilitate encoding of patients diagnosed in ITIS as a proof that the patient has been notified to the Department of Health.6. Prepare a monthly progress report for the assigned area on the number of cases reported and other indicators, related to TB screening, testing & diagnosis, treatment and prevention as per implementation matrix and indicators vis a vis target including the facilitating and hindering factors to good performance which will be the basis for prioritizing the next cycle of monitoring visit.7. Validate raw data collected using the data extracted by the Monitoring & Evaluation Department, identify discrepancy and reasons as well as resolve discrepancy issues.8. Performs other relevant duties as requested by the Management, through the Project Coordinator.QUALIFICATIONS, KNOWLEDGE, AND SKILLS REQUIREMENTS1. Bachelor's Degree in any Allied Health Course2. Good Communication Skills3. Problem Solving ability4. Preferably with experience working in a hospital setting, Department of Health, Local Government Unit (LGU), NGOs and other stakeholders.5. Computer literate and with experience in maintaining a data base6. Ability to work well in a team and has good interpersonal relationship7. Ability to work under pressure8. Ability to work independently with minimum supervision9.
