Coordinates patient’s discharge planning needs
Ensure that patients will not receive unnecessary procedures,ineffective treatment, or unnecessarily extensive hospital stays.Maintain accurate records of all patient related interactions
Minimum QualificationsUSRN with an active or inactive license
